We are seeking a dedicated and passionate Learning Support/EHCP Transition Coordinator to support young people with Special Educational Needs and Disabilities (SEND) as they transition into post -16 Education.
In this pivotal role, you’ll work closely with young people, families, schools, and professionals to ensure smooth, tailored transitions into college for learners with SEND, particularly those with Education, Health and Care Plans (EHCPs).
